Youth Evaluation Services
To receive an evaluation: |
- Call 266-8666 (Prevention and
Recovery Services) as soon as possible after a referral to
make an appointment. There may be a waiting period.
- A two hour appointment will be
scheduled to complete an evaluation. Please arrive 20
minutes before the appointment to complete the paperwork, and
sign confidentiality and release forms.
- Confidentiality is required by
state and federal law. No information obtained
either verbally or in writing will be released to any other
individual or agency without a release signed by the youth and
parent.
- While both parents or legal
guardians are encouraged to attend the evaluation,
at
least one parent or legal guardian must accompany the youth to
the session. (Documentation of legal guardianship
must be provided at the time of appointment if person
is not the natural parent.) The evaluation process
will include time with the youth and counselor only; with the
parent or guardian and counselor only; and with the youth,
parent or guardian and counselor.
- If the evaluation is a result
of a school suspension, a school official designated by the
student and parent will be informed by letter that the
evaluation has taken place. Recommendations made by the
counselor will be released
only to individuals specified by the youth or parent.
- A recommendation will be made
after an evaluation is completed.
- A follow-up survey will be
sent to determine if recommendations were helpful or if
additional services are needed.
